Outdoor Activity Space — Based on LPA observation, the licensee did not comply with the section cited above in that 3 trIcyles were not maintained in good working order, plastic tires were coming off of the wheel and rubber matting under play structure is ripping apart which poses/posed a potential health, safety or personal rights risk to persons in care.
