Responsibility for Providing Care and Supervision for Infants — LPA observed an infant teacher leave the infant classroom and enter the another classroom to assist a parent dropping their infant child. In doing so, the infant teacher left four infant alone in the classroom. This poses an immediate health, safety or personal rights risk to persons in care

Admission Agreements — Based on interviews and record review, the licensee did not comply with the section cited above. In the course of the investigation, it was determined that some parent/legal guardians had been informed that the facility closes at 5:30pm and/or earlier than 6:00pm, which differs from the parent handbook and facility license, which state 6:00pm. This poses/posed a potential health, safety or personal rights risk to persons in care.

In reviewing staff records, staff #10 was hired with a TB test/Health Screening that had been conducted over a year ago.
