Staff immunization — influenza, pertussis and measles — Based on the review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the staff file review, the LPA was unable to review immunization records for S2.

Family home — home free of safety hazards — Based on the record review, the Licensee did not meet the above regulation which poses a potential health and safety risk to the children in care. During the review of facility documents, the Licensee stated she has a facility roster, however, she was unable to locate it during the inspection.

Infant Safe Sleep — During the children’s file review the LPA reviewed sleep logs for C1, C2, C3 that were timed for every 30 minutes. All sleep logs had set times such as 12:15PM, 12:45PM, 1:15PM, etc.
