Responsibility for Providing Care and Supervision — LPA observed Child 1 climbing a book case and eating the tip off a marker without teacher's noticing. It was revealed that Child 1 was eating sand from the sandbox when under staff supervision. This poses a potential health, safety and personal rights risk to children in care.

Personal Rights — Child 1 was receiving diaper rashes while in care as well as wearing incorrect diapers. Parents share same concern of their child receiving diaper rashes and diapers that do not belong to the child. This poses a potential health, safety and personal rights risk to children in care.
