At this time, there is no child enrolled that requires regualr presonal care with the assistance of water. This poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on interview and record review, the licensee did not comply with the section cited above in that 1 out of 3 staff files reviewed, Staff 2 (S2) did not have a health-screening report available to review. This poses/posed a potential health, safety or personal rights risk to persons in care.

This structural change that temporarily reduces the outdoor activity space, was not reported to the Department prior to making the change. This poses/posed a potential health, safety or personal rights risk to persons in care.
