Responsibility for Providing Care and Supervision — Based on interviews conducted, documentation reviewed, and observations made, it was revealed that the facility failed to provide adequate supervision for the two subject children, due to them being to leave the facility.

Criminal Record Clearance — Based on LPA observation and record review, S1 was working in the preschool classroom without a completed fingerprint clearance which poses an immediate health and safety risk to persons in care.

LPA observed that Mandated Reporter Training Certificates were expired for S3, S5, and S6.
