Reporting Requirements — There was a change in director that was never reported to the department. The acting director has been present in her position since January 2026, and this was never reported to the department. This poses a potenital risk to the health an safety of children in care.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above. In all but one of the preschool classrooms, chemicals were found behind cabinets that were not kept locked. Any harsh chemicals that mention "poison control" on them are considered poisonous and must be kept behind a key lock. This poses/posed a potential health, safety or personal rights risk to persons in care.
