Infant Care Teacher Qualifications and Duties — Based on the observation and record review, the Licensee did not meet the above regulation which poses an immediate health, safety and personal rights risk to the children in care. During the facility tour the LPA observed S1 and S2 providing Care & Supervision to seven infants. After reviewing the staff files, the LPA was unable to review complete transcripts with required classes for S1or S2. S1 did not have any transcripts and S2 did not have Infant/Toddler credits.

Responsibility for Providing Care and Supervision for Infants — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the facility tour the staff acknowledged C4 can roll from their stomach to back on their own. During children’s file review the LPA was unable to review an LIC 9227 with Section C completed.

Modifications to Infant Needs and Services Plan — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review current Infant Needs & Service Plans for C1 and C3.

Infant Care Teacher Qualifications and Duties — Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that of the 6 staff present in the two infant classrooms, only 1 staff was determined to be a qualified teacher, while the other staff have certificates on file whose accreditation could not be validated at this time which poses a potential health, safety or personal rights risk to persons in care.
