Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above. S1 did not have proof of MMR (measles) on file for review. Although there was a written statement stating immunity from a licensed physician from another country, Title 22 does not accept health information from any physician, let alone another country for this vaccine. This poses/posed a potential health, safety or personal rights risk to persons in care.

Upon todays inspection, a full file review for director was conducted. Missing items were discussed with director (see in POC statement). Fixtures, Furniture, Equipment and Supplies — Based on observation, the outdoor play area had multiple large screw-like parts sticking up more than one inch, holding down the small gate surrounding the playground to the right. There were also multiple play structures with cracks, exposing sharp plastic. There is one out of two small benches with a steering wheel which has a large exposed wood piece that the children sit on. This poses/posed a potential health, safety or personal rights risk to persons in care.
