Infant Care Activities — The licensee did not comply with the section cited above in that during the tour at 8:59AM LPA observed a 3 month old infant sleeping on ground with attached pacifier which poses an immediate health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above. S1 did not have proof of MMR (measles) on file for review. Although there was a written statement stating immunity from a licensed physician from another country, Title 22 does not accept health information from any physician, let alone another country for this vaccine. This poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on record review, the licensee did not comply with the section cited above. The center had a change in director and never informed licensing of this change. Upon todays inspection, a full file review for director was conducted. Missing items were discussed with director (see in POC statement). This poses/posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — Based on record review and staff admission, there was no record of infant 15 minute sleep logs on file. Staff admitted that they could have been tossed out. Facilities are required to keep documentation on file for 3 years. This poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above. While requesting documentation in the infant classroom, a staff stated that the 15 minute sleep log could have been tossed. The log is a required document to keep on file for up to 3 years. During today's inspection, there were no logs available to review. This poses/posed a potential health, safety or personal rights risk to persons in care.
