Staff-Infant Ratio — Based on interview and record review, the Licensee did not comply with the section cited above, the staff-infant ratio was not within 1:4 (one staff to 4 infants) the week of August 28, 2023, which poses an immediate Health and Safety, Personal Rights risks to persons in care.

Limitations on Capacity and Ambulatory Status — Based on LPA observation, the Licensee did not comply with the section cited above on 09/05/2023, LPA observed a total of 26 infants in the infant classrooms and the maximum capacity is 24 infants, which poses an immediate Health and Safety, Personal Rights risks to persons in care.

Criminal Record Clearance — Based on LPAs observation and record review, S1 was working in the infant classroom without a fingerprint clearance and/or being associated to the facility, which poses an immediate health, safety or personal rights risk to persons in care.

Personal Rights — Based on the interview and record review, the Licensee did not meet the Personal Rights regulation which poses an immediate Personal Rights risk to the children in care. There were at least 13 incidents documented involving a child being bitten, scratched and/or hit. The Interim Director confirmed the incidents reported did occur while in care.
