Reporting Requirements — Based on interview and record review, the licensee did not comply with the section cited above in that the facility did not report an incident on 7/15/2024 where a facility van damaged a fence and the door of classroom 104. This poses/posed a potential health, safety, or personal rights risk to persons in care.

When opened there was a label on the inside of the panels that stated "Danger hazardous voltage contact will cause severe injury or death."
