Health-Related Services — Based on interview and record review, a child fell and injured their leg on the playground. The child had difficulty standing and showed signs of limping and of being injured. Although the child showed signs of injury no medical attention was sought out for child. This poses/posed an immediate health, safety, or personal rights risk to persons in care.

Personnel Requirements — Based on observation and interview, the licensee did not comply with the section cited above in that there was 1 volunteer with current criminal record clearance or exemption that was not supervised while providing care and supervision to children in care. Personal Rights — Based on interview & record review, the facility did not prevent a child in care from being "smacked" on the butt with a toy, multiple times, by a temporary staff member. Buildings and Grounds — Based on observation the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed during the tour in one of the classrooms a mop with cleaning solution attached to it was on the floor in the bathroom and accessbile to the children. LPA observed no children were in the bathroom at the time of inspection.
