Plan of Operation — Director stated that she has been using the new classrooms 110 and 112 since Monday 08/19/2024 prior to licensing's approval as the elementary took over their other classrooms. This poses a potential health and safety risk to children in care.

Written directives for drinking water lead testing — On 1/5/23, Licensee failed to maintained a lead value at or below the Action Level for water lead testing resulting with values of 5.5 ppb or greater for outlet "A". Water testing results identified with Action Level Exceedance as defined in WD section 101700.3 are not deemed safe to drink. This poses a potential and health and safety risk to persons in care.
