Responsibility for Providing Care and Supervision — Based on interviews during play yard time, a child wandered into an off-limit area of the building which is not part of the licensed facility without the knowledge of the staff.

Outdoor Activity Space — Based on observation,, the licensee did not comply with the section cited above where during the walk through, LPA observed a large ladder in the childerns outdoor activity space, DIR removed during inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
