Responsibility for Providing Care and Supervision — Based on interviews, the licensee did not ensure care and supervision of children in care since after-school staff did not realize a child had been left unattended in the facility transportation van for approximately 45 minutes, which posed an immediate risk to the health and safety of children in care.

Food Service — Based on observation, during the tour of the facility, LPA observed the licensee did not comply with the section cited above where 2 bottles of bleach, 1 box of ant killer and 1 can of raid was stored in the sink cabinets which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in maintaining the wall of the school age classroom that has a hole and teh electric outlets located in the classroom do not have a childsafe cover which poses a potential health, safety or personal rights risk to persons in care.
