Buildings and Grounds — Based on observation the licensee did not comply with the section cited above in where during the tour of the facility LPA observed Rm 2, Rm 4 changing tables with dirt/grime, floor boards and walls with dirt/grime Rm 1-4, 5 outlet covers missing between Rm 1-4, which poses/posed a potential health, safety or personal rights risk to persons in care.
