Reporting Requirements — Based on record review, and interviews, C1 obtained medical treatment due to an injury that occurred at the CCC. CCL was not notified via telephone and was notified in writing, twenty days after medical attention was obtained. This failure to provide timely notification poses a potential risk to the health, safety, and personal rights of children in care.
