Responsibility for Providing Care and Supervision — ; Based on written reports, file review and interview, on 07/27/2026 a child attending the CCC became aggressive towards classmates and staff, eloped from the facility grounds, into the neighboring apartment complex. The CCC had knowledge of the child’s history of behaviors prior to start date and did not have a plan in place to support the needs of the child and classroom, which posed a potential health & safety risk to children in care.

Responsibility for Providing Care and Supervision — , Based on interviews conducted 2 of 4 staff and confidential interviews revealed C1 was observed to be left alone in the office on 4/4/2025. It was not disclosed the length of time, but it was stated that multiple times C1 was left alone so the Director could prepare lunch in the kitchen and grab a child off the bus from outside. This is a potential risk to the health & safety of children in care.
