Responsibility for Providing Care and Supervision — Staff not having visibility of child when being in dipper changing area (blind spot.) Based on observations today it was evident that changing station door creates a blind spot and door lock is accessible to children.

Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above. The facility has not conducted a fire drill since May 2024. This deficiency poses a potential health, safety or personal rights risk to persons in care.
