Infant Care Activities — infant in care was allowed to sleep on their stomach for approximately three hours. This was documented on Kindercare app sleep log. This is an immediate threat to the health and safety of children in care.

Food Service — Based on interviews and record review, the director did not comply with the section cited above, as children were served food from a bowl that had been contaminated which poses a potential health, safety, and personal rights risk to children in care.
