Personal Rights — During staff interviews, it was discovered that previous staff member (F1) would yell and make inappropriate comments towards children in care. These actions constitute a failure to treat children with dignity and respect. This is a potential health, safety, and personal rights risk to children in care.

Personal Rights — Based on staff interviews, it was disclosed that previous staff memeber (F1) was observed calling children in care "ugly" and yelling statements like "shut up" and "be quiet". LPA determined that this is a potential health, safety, and personal rights risk to children in care.

Food Service — Based on investigation interviews, partially used prepared infant formula bottles have periodically been saved and refed/offered later in the day. Per CDC, "Throw out any infant formula that is left in the bottle after feeding your baby. The combination of infant formula and your baby’s saliva can cause bacteria to grow." By feeding left over prepared formula this poses/posed a potential health, safety or personal rights risk to children in care.
