The date by which the licensee must file CCLD's Proof of Correction form, certifying under penalty of perjury that the deficiency was corrected on or before that date. Filing it does not prevent CCLD from making a follow-up visit to verify.

Personal Rights — Based on staff interviews, it was disclosed that staff (S8) physically restrained a child (C1) and held them down in their cot against there will. LPA determined that this is a potential health, safety, and personal rights risk to children in care.

Responsibility for Providing Care and Supervision — Based on staff interviews, it was disclosed that on 4/30/26 children (C2) & (C3) left the supervision of the classroom for approximately 2 minutes. LPA determined that his is a potential heatlh, safety, and personal rights risk to children in care.
