Health-Related Services — Based on observation, interview, and record review, the licensee did not comply with the section cited above in that there were four (4) epic pens in baggies that did not contain a label or expiration date which poses an immediate health, safety or personal rights risk to persons in care.

Infant Care Center Sleeping Equipment — Based on observation and interview, the licensee did not comply with the section cited above in that four (4) infants who could climb out of cribs did not have a mat or cot to sleep on which poses/posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — Based on observation, interview, record review, the licensee did not comply with the section cited above in that the documentation recorded for infants safe sleep did not include position infant was sleeping in or infant's condition which poses/posed a potential health, safety or personal rights risk to persons in care.
