Responsibility for Providing Care and Supervision — Based on LPA’s interviews, the Licensee did not comply with the above by leaving a child unsupervised in the facility’s outdoor play area. This posed a potential risk to the health, safety, or personal rights of persons in care.

Personal Rights — Based on interviews and observations made children are placed in high chairs while not actively eating for extended times as well as the facility uses the chairs for activities (arts and crafts projects) other than feeding. This poses a potential health and safety risk to children in care.
