Responsibility for Providing Care and Supervision for Infants — Upon review of the infant sleep logs report from bright wheel, staff documention of 15 minute checks from the time the child falls asleep till the time the child is awake from their naps is not completed with some days no documentation of 15 minutes sleep checks at all.

(b) There shall be a ratio of one teacher for every four infants in attendance... Each aide is responsible for the direct care and supervision of a group of no more than four infants. Based on LPA's observations and interviews conducted, it is evident that the facility was not in compliance due 1 teacher and 1 teacher aide supervising 9 infants(not asleep).

101229.1 Sign In and Sign Out... (b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. Based on observation and document review... the Center did not comply with the section cited above, on 7/17/2024 C31 signed in at 9:14AM, no sign out was registered. On 7/19/2024 C27 signed in at 8:56AM no sign out was registered. Lastly on 10/03/2024 C38 was signed in at 8:38AM no sign out was registered.
