Infant Safe Sleep — Based on observation, interview, and record review, the licensee did not comply with the section cited above, which poses an immediate health, safety, or personal rights risk in persons in care. LPA observed one infant (C1) swaddled and placed on a raised cushion and restricted by velcro while sleeping inside a crib.

During this time, FCCH had 7 children of which 3 were infant age which is an immediate risk to the health, safety or personal rights risk to persons in care.

Licensee stated she did not have all children listed on the roster, which LPA reviewed to be incomplete.
