Sign In and Sign Out — Per review of the sign in and out records, some children have been signed out in advance for today and some authorized representatives have included the sign in and out time but not their signatures. This poses a potential risk to the health, safety and personal rights of children in care.
The date by which the licensee must file CCLD's Proof of Correction form, certifying under penalty of perjury that the deficiency was corrected on or before that date. Filing it does not prevent CCLD from making a follow-up visit to verify.
Sign In and Sign Out — Per LPA's review of the sign in and out records, 41 school age children were present but only 19 authorized representatives signed children in today. LPA also observed some children have been signed out in advance for today and future dates. Some parents were observed to have included the sign in and out time but not their signatures. This poses a potential risk to the health, safety and personal rights of children in care.

Personal Rights — Based on staff and children interviews, 4 witnesses who were interviewed stated staff held C1 down. 1 stated staff was grabbing C1 by the arm. Based on photographs obtained of child, C1, marks are visible on the upper right arm from this incident. This poses a potential risk to the health, safety and personal rights of children in care.

Staff immunization requirements — Based on observation, interview and record review, the licensee did not comply with the section cited above for two staff (S1 and S2) which poses a potential health, safety or personal rights risk to persons in care. Through file review LPA observed that staff S1 does not have proof of immunization for pertussis and staff S2 did not have immunization proof for measles and pertussis.
