Teacher Qualifications and Duties — Based on LPA's observation and record review, the licensee did not comply with the section cited above in 2 out of 8 staff which poses a potential health, safety or personal rights risk to persons in care. Staff (S1 & S2) are currently utilized as teachers but they do not meet the required qualifications.

Admission Policies — LPA learned from interviews that facility staff have been allowing a parent (P1) to visit child (C1) during daycare hours under staff supervision; this violated the court order. This poses a potential risk to the health, safety or personal rights to children in care.

LPA observed that a child walked out of the restroom in Room 4. There was no staff standing at the doorway were they can visually supervise the child.
