Personnel Requirements — Based on observation, the licensee did not comply with the section cited above during the facility tour of the toddler classroom LPA observed 7 toddlers and one staff (S2) in classroom number 1 which posesa potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — Based on interview and record review, the licensee did not comply with the section cited above staff stated that they do not complete 15 minute physical sleep check which poses a potential health, safety or personal rights risk to persons in care.
