Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above. LPA observed two infants asleep with blankets and/or pillows in their play yards which poses an immediate health, safety or personal rights risk to persons in care.

LPA observed licensee added a day care room to the home and did not notify the Department which poses/posed a potential health, safety or personal rights risk to persons in care.
