Drinking water lead testing — Based on interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Facility has not completed the required water lead testing by the deadline.

Personnel Requirements — Based on observation, interview and record review, the licensee did not comply with the section cited above for three staff, S2, S3 and S4 which poses a potential health, safety or personal rights risk to persons in care. Staff, S2, S3 and S4 were observed to be missing the required health screening report, Staff S3 and S4 were missing proof of negative tuberculosis test and staff S5 was missing proof of immunization's for measles and pertussis.
