Criminal background check — Based on observation, interview and record review, the licensee did not comply with the section cited above for staff, S1 which poses a potential health, safety or personal rights risk to persons in care. One staff, S1 was observed to be cleared but not associated to the facility. Director reviewed Guardian and observed a request for additional verification. Director completed the association during today's inspection. LPA reminded Director to verify staff association's prior to staff being present or working at the facility.
