Responsibility for Providing Care and Supervision — LPA observed Teacher (S1) in the children's bathroom helping children wash their hands and Teacher's aide (S2) squatting down, engaging with children lined up by the classroom door with her back turned to a child on the carpet behind her and two children playing with a diaper bin. This poses a potential risk Health, Safety Personal Rights risk to children in care.

Responsibility for Providing Care and Supervision — Based on interviews, infant/toddler and preschool staff failed to observe child, C1 leave one classroom and join another. Teachers did not count children and conduct name to face checks as part of transition protocols. Teachers gave LPA varying accounts of where child was found and who escorted child back to their classroom. This poses a potential risk to the health, safety and personal rights of children in care.
