Buildings and Grounds — Based on observations and interviews, the facility did not maintain the premises in good repair. The facility experienced periods of no running water, non flushing toilets, and insufficient water pressure on May 14, May 15, and May 22, 2026. A children's toilet was also found non operational for an extended period. These conditions impacted toileting, diapering, and required hand washing, posing a potential health and safety risk to children in care.
