Personal Rights — Based on interview the licensee did not comply with the section cited above. Licensee admitted to swatting a childs hand, which poses a threat to the child's health, safety, or personal rights risk to persons in care.

Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above in that she did not have a crib or play yard in the facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on observation and interview, the licensee did not comply with the section cited above in two out of two children's records. LS did not have immunization records available for C1-C2 which poses a potential health, safety or personal rights risk to persons in care.
