Infant Care Center Sleeping Equipment — Based on interviews with LS, D1 & S1 admitted the facility allowed infants under 1 year old to sleep on cots/mats. Also LPAs observed an infant on a cot/mat which poses a potential health, safety, or personal rights risk to persons in care.

Infant Care Food Service — Based on interviews with LS and D1 amitted staff use boppy pillows to assist staff to prop up infants when feeding milk bottles to them, which poses a potential health, safety, or personal rights risk to persons in care.
