Responsibility for Providing Care and Supervision — Based on interviews conducted, Director Zettler’s admission, and information obtained during the investigation, the Department determined that the facility did not provide adequate supervision of children, resulting in children being able to leave the classroom and building without staff supervision. This poses an immediate health and safety risk to children in care.

Criminal Record Clearance — Based on interview and record review, S1 was working with children at the facility without an associated criminal record clearance, which poses an immediate health & safety and personal rights risk to the children in care.

Responsibility for Providing Care and Supervision — Based on the director's admissions, staff interviews, and information obtained during the course of the investigation, it was determined that there were multiple instances in which children were not provided with adequate supervision while in their classroom. Thisposes a potential risk to the health and safety of children in care.

Responsibility for Providing Care and Supervision — Based on interviews conducted by the LPA and the center director’s own admission, it was corroborated that on 04/02/26 at approximately 09:30am, child C1 was without supervision of a teacher for approximately two minutes while in the classroom before being reunited with their class. This posed a potential health, safety or personal rights risk to persons in care.

Criminal Record Clearance — Based on a review of the personnel report dated April 10, 2026, and observations made during the visit, Staff S1 was observed working directly with children in care. However, S1 was not listed on the facility’s personnel roster and does not have an eligible California clearance on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on an interview with the facility’s new center director, Mario Zettler, on February 18, 2026, it was confirmed that the former center director, Mackenzie Miller, is no longer employed at this facility, with her employment ending in November 2025. The Department was not notified of this change. This poses a potential risk to persons in care.

Responsibility for Providing Care and Supervision — Based on the center director's admission on 02/12/25 and the Unusual Incident Report (UIR) submitted to CCLD on 01/22/25, on 01/17/25 at approximately 06:00pm, child C1 was left without supervision of a teacher for approximately one minute while in a preschool classroom.This presents a potential health and safety risk to the children in care.
