LPA observed children using the restroom without supervision on three separate occasions during the facility inspection. Teacher Qualifications and Duties — Based on staff record review, the licensee did not comply with the section cited above. LPA observed that Lead Teacher of the Toddler Component and Facility Director were missing 3 units in the infant/toddler category which poses a potential health, safety or personal rights risk to persons in care.
