Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in that S1, S3-S5 were missing immunization record of Measles, Pertussis and Influenza which poses a potential health, safety or personal rights risk to persons in care.

Teacher Aide Qualifications and Duties — Based on observation, interview, and record review. Upon LPA's arrival to the center there were two staff with 0 units. One staff was left alone with either 4 or 7 children. LPA could not verify which aide was left alone with children and which aide was under the direct supervsion of Qualified teacher. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on observation, interview, and record review, Three or more children were missing signitures and time when they were taken from the facility on two or more dates for December 2023. LPA showed the sign in/out sheets to the licensee. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
