Teacher #2 was associated with the facility but not with the preschool program; she is currently associated with the infant program (license #197418426). which poses an immediate health, safety or personal rights risk to persons in care.

During today's visit, LPA reviewed 5 children files and only 2 children have TB risk assessments, which poses/posed a potential health, safety or personal rights risk to persons in care.

During today's visit, LPA observed a trash can with food inside without a cover, which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above. There are 2 staff were unable to provide their Immunization record for their Measles and pertussis, which poses/posed a potential health, safety or personal rights risk to persons in care.
Health-Related Services — Based on interview and record review, the licensee did not comply with the section cited above. There is a child need IMS service, however, the facility does not have IMS plan for the child, which poses/posed a potential health, safety or personal rights risk to persons in care.
