Drinking water lead testing — Based on interview, the licensee did not comply with the section cited above. During today's visit, the facility does not complete the Water Lead Testing. According to the owner, she will submit the requrest to the water testing agency to schedule an appointment. which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above. LPA observed 4 teachers and 3 of teachers do not have MMR or Dtap,which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above. During today's visit, LPA reviewed 5 children's files. 3 children do not have physical exam or TB risk assessment, which poses/posed a potential health, safety or personal rights risk to persons in care.
