LPA observed that Lead Teacher of the Toddler Component and Facility Director were missing 3 units in the infant/toddler category which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — observation. LPA Marquez observed a wooden gate with a broken/loose latch. This poses a potential risk to the health, safety and personal rights to children in care.
Mandated reporter training — Based on a record review, the licensee did not comply with the section cited above. Three of the thirteen staff files are missing the required Mandated Reporter Training (MRT) which poses a potential health, safety or personal rights risk to persons in care.
