Buildings and Grounds — Based on observation upon entering the inside preschool side of CCC, LPA observed clutter of toys, materials, furniture, supplies stacked and placed throughout the entrance which poses/posed a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation the licensee did not comply with the section cited above in 2 identifiers where upon entrance to the outdoor activity space, LPA observed a pair of dirty child sized underpants at the front gate and Inside the center of the outdoor activity space, wooden beam approx. 3.5 feet in length and worn cardboard boxes placed as ground covering were observed. which poses/posed a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation the licensee did not comply with the section cited above in 3 identifiers where LPA observed a wooden Garden box containing 3 rusted screws protruding from the corners, a Rusted pipe with 3 ends tied to a rope hanging from fence play yard fence, reachable by children and a baby gate zip tied to top of play structure to prevent use of slide, had a large hole in the center of the plastic with jagged edges, which poses/posed a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — Based on observation the licensee did not comply with the section cited above in 1/1 counts where LPA's observed S4 alone with 6 children in outdoor activity area, using a hand held device, not supervising children in care which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on observation, interview and record review, the licensee did not comply with the section cited above in 1/1 teachers (S4) that are alone and supervising children did not have a current CPR/1st Aid on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Food Service — Based on observation, the licensee did not comply with the section cited above when LPA observed 2 cleaning products on kitchen counter of food preparation area that is used to prepare snack/meals for children in care which poses/posed a potential health, safety or personal rights risk to persons in care.
Napping Equipment — Based on observation the licensee did not comply with the section cited above in 9/9 nap mats in the preschool classroom had ripped/torn/exposed foam in nap mats used by children in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
