Sign In and Sign Out — , 2nd violation, on LPA's observation at 9:50 am, 3 of 5 infants Sign In for the day did not have a full legal signature of the parents and 1 out of 5 infant was not signed in, the licensee did not comply with the section cited above in which poses an immediate risk to the health, safety or personal rights to children in care.
Infant Care Activities — 101430 Infant Care Activities (a) Notwithstanding Section 101230, the following shall apply: 3) All infants shall be given the opportunity to sleep without distraction or disturbance from other activities at the center whenever the infant desires. (E)If an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible. Based on the above LPA's observation, at 9:46 am, LPA Estoesta and LPA Jackson oberserved 2 infants (C1 and C2) were sleeping in their respective car seats at the Infant Room floor, next to the cribs. LPA asked Staff Rgbe Haile, "How long they have been sleeping?" Staff stated since they "got here." LPA Estoesta verified today's sign in for C1, no record time but signed by a parent, and C2 sign in was at 6:40 am but no parent signature., the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA Jackson observation, at 11:37 am, 3 infants were napping in their respective cribs with blankets and pillows on the infants, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care.

Child Care Center Director Qualifications and Duties — Based on observation and interview, the licensee did not comply with the section cited above. The director, Merat, was not present at the facility and it was stated that she is rarely at the facilty, which poses a potential health, safety or personal rights risk to persons in care.
Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation and interview\, the licensee did not comply with the section cited above. It was found that the changing tables are covered in disposable toileting pads instead of plastic or washable vinyl, which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision for Infants — Based on interview and record review, the licensee did not comply with the section cited above. The facility stated that they were not aware of the infant sleeping logs and do not have sleeping logs for the infant in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Food Service — Based on observation and interview, the licensee did not comply with the section cited above. The facilty did not properly label milk for a child in care which poses a potential health, safety or personal rights risk to persons in care.

Child Care Center Director Qualifications and Duties — , on LPA's interview, the Licensee claimed that she has assigned a qualified director on her other centers and record says that her name showed a a director on the other site and on LPA's arrival, on multiple visits, the Licensee was not present, the licensee did not comply with the section cited above in which poses a potential risk to the health, safety or personal rights risk to children in care.
Responsibility for Providing Care and Supervision for Infants — , 2nd violation, on LPA's observation at 10 am, the Licensee have "Baby's Daily Report Sheet" but the Staff did not document their condition to check for signs of distress, which includes, but is not limited to labored breathing, flushed skin color, increase in body temperature, and restlessness. The licensee did not comply with the section cited above in which poses an immediate risk to the health, safety or personal rights to children in care.

Infant Needs and Services Plan — 101419.2 Infant Needs and Services Plan (a) Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant.
