Reporting Requirements — Based on record review, and interviews, CCC was notified of a second confirmed case of Hand, Foot, and Mouth Disease; however, notification was not provided till next day. Furthermore, nine families were not informed of the outbreak and did not receive the notification letter. This failure to provide timely notification poses a potential risk to the health, safety, and personal rights of children in care.
Napping Equipment — Based on observation, the licensee did not comply with the section cited above in that the sleep mats did not distinguish between sleeping side or floor side. LPA's also observed floor mats with exposed foam which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in that there were hazardous items labeled "Keep out of the reach of children" in the storage cabinets and closets in the classrooms that were accessible to children in care which poses a potential health, safety or personal rights risk to persons in care.
