Criminal Record Clearance — (2) Request a transfer of a criminal record clearance as specified in Section 101170(f) Based on observation, interview and record review, the licensee did not comply with the section cited above for staff, S4, which poses an immediate health, safety or personal rights risk to persons in care. LPA observed staff, S4, working with children but without having fingerprint clearance transferred to this facility.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 2 out of 5 staff files were missing Mumps, Rubella or an Influenza Vaccination/Declination which poses a potential health, safety or personal rights risk to persons in care.

Napping Equipment — LPA observed children's used blankets in contact with other bedding on cots stacked. Based on observations, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on staff file review, LPAs observed staff, S1 does have a health screening report (LIC 503) on file.

101220 Child's Medical Assessments (a) Prior to, or within 30 calendar days following the enrollment of a child, the licensee shall obtain a written medical assessment of the child. This medical assessment enables the licensee to assess whether the center can provide necessary health-related services to the child. This requirements is not met evidenced by: Based on children's file review LPAs observed child C6 is missing the physician's report.

Responsibility for Providing Care and Supervision — During facility walk through, LPAs observed a child using the bathroom in the 3-4 year old room without staff supervision.

Health-Related Services — During medication review, LPAs observed child, C7 does not have an epinephrine auto injector in original container.

Personnel Records — Based on observation, interview and record review, the licensee did not comply with the section cited above all staff, which poses a potential health, safety or personal rights risk to persons in care. Facility representative did not have access to personnel records in the absence of the director at the facility.

Child's Records — Based on observation, interview and record review, the licensee did not comply with the section cited above all children in care, which poses a potential health, safety or personal rights risk to persons in care. Facility representative did not have access to children's records in the absence of the director at the facility.
