Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above in the fire extinguisher was last serviced on 6/5/23 which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation and record review, the licensee did not comply with the section cited above in licensee and assistants do not have current mandated reporter certification which poses a potential health, safety or personal rights risk to persons in care.
