During file review, the LPA discovered that children 1 - 3 were missing the following Licensing forms from their facility files; LIC 613A / Personal Rights, LIC 700 / Identification & Emergency Information, LIC 701 / Phsycian's Report, LIC 995 / Notification of Parent's Rights, & LIC 627 / Consent for Emergency Medical Treatment. This poses a potential health, safety or personal rights risk to persons in care.
