Outdoor Activity Space — Based on observations the licensee did not comply with the section cited above by having a large gap in the fencing area of the playground leaving open space for exiting the playground area leading to hazardous space. which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — LPA observed three (3) sleeping infants behind a closed door in the nap room with no teacher present in the room with them, which poses an immediate health, safety or personal rights risk to persons in care. ABSENCE OF SUPERVISION/ CIVIL PENALTIES ASSESSED.

Admission Policies — staff 1 did not allow parent entry into a classroom in a timely manner. This is a potential risk to the health and safety of children in care.
Infant Care Center Sleeping Equipment — Based on observations the licensee did not comply with the section cited above by having 5 cribs with sheets that were not appropriate to the crib mattress which poses/posed a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on ibservations the licensee did not comply with the section cited above by having a changing station door unit broken and missing one bottom cabinet which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on the licensee did not comply with the section cited above, LPAs observed one large item that appeared to be a mascot object used as climbing structure placed on the cement which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Activities — Based on observation and record review the licensee did not comply with the section cited above by not having LIC 9227 form in unfants that are 12 months or younge which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observation record review, the licensee did not comply with the section cited above by not having all required immunizations for staff present which poses/posed a potential health, safety or personal rights risk to persons in care.
